Job Location Cullman Regional Medical Center - Cullman, ALPosition Type PRN - .2

Evaluates and admits patients to Home Care, develops plan of care and provides care to patient

according to plan

Acts as case manager making necessary adjustments in patients plan of care related to symptom changes

and changes in care needs.

Reviews and plans patient care and or discharge during scheduled case conference meeting and

documents accordingly.

Acts as Liaison for pt/caregiver with MD and community health care partners

Strives to lower cost of care through managing mileage, supplies and visit scheduling.

Completes all documentation related to pt contact; assessments; SOC; Recerts

Resumption of Care and Discharge according to policy

Maintains understaing of current Home Care Regulatory Guidelines

Provides on call coverage for both Home Care and Hospice as assigned

Demonstrates and encourages team behavior and exceptional patient/guest experiences.

Upholds and promotes patient safety and quality.

Qualifications

Minimum Knowledge, Skills, Experience Required

Education

Associates degree in Nursing and currently licensed by the State of Alabama required.

Experience

Minimal 2 years experience in a hospital setting working in Med/Surg; Critical Care or ER. Previous Home Care or Hospice experience preferred.

Additional Skills / Abilities

Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize schedule to meet patient needs.
